transition

In Arizona, the high-school transition planning takes place during a student’s IEP no later than the school year when the child turns 16. The transition plan is designed as a results-oriented process with the focus on measurable post-secondary goals. The team considers the student’s academic progress and the progress in their coordinated set of transition activities including but not limited to: community experiences, the development of employment, and when appropriate, acquisition of daily living skills.
